By submitting this form, I hereby confirm that I HEREBY voluntarily accept the Terms and Conditions of the services provided by the Clinic and I am fully aware of what these services involve.
I hereby/the client acknowledges that the Clinic does not provide any guarantee or warranty as to the success of any suggestions, protocols or products provided by it or its staff; and client further agrees to legally release the clinic and its staff of any responsibility for the healing process and possible legal action. This means you release, waive, discharge and covenant not to sue the Clinic and staff from any liability, claims, demands, actions and causes of actions whatsoever related to any loss, damage, injury (including death) that may be sustained in the course of the services provided or thereafter.
I hereby/ the client accept full responsibility and liability for the services provided to me and for which I have engaged the Clinic as well as for any consequences thereof or effects related thereto whatsoever.
I hereby/ client exempt the Clinic (Ashshifaa Services Ltd) as well as its staff and any authorized representative thereof from any liability in any form or manner for any consequences or effects whatsoever from any of the services provided to me, whether emotional, mental or physical that may occur or be experienced during or after this/these session(s) or from any damage or any personal or psychiatric injury that may be sustained at the premises of the Clinic or not on premises, or during the course of treatment.

The client hereby agrees to and/or consents to the services provided by the Clinic by signing the consent form below prior to any consultation/appointment/psychotherapy/counselling/prophetic Sunnah treatment or any other treatment or service provided.
The Clinic does not provide medical diagnoses, and/or consultations related to health, medical, or psychiatric conditions or issues. The Clinic and its staff are hereby exempted from all liability for any statements, representations or warranties that may be misconstrued as medical or clinical advice for any such conditions or issues.

Spiritual Treatment and Alternative Medicine
Ruqya is a form of spiritual treatment which solely entails use of quranic recitation onto oneself or onto products for consuming or applying to one’s body. Hijama (wet cupping) is a spiritual, religious, natural, alternative means of treatment. Certified hijama practitioners are not licensed medical doctors. Please remember the cure is from Allah alone, if your treatment is not working, please check yourself. It can be that there are prohibited things present in your life. e.g., taweez, guards, charms, idols, things for protections, patient is committing sins not praying, eating/drinking things that are prohibited according to Islam, indulging in prohibited money according to Islam, adultery or fornication.
